Introduction to W-9 Form

Key purpose

To provide your Taxpayer Identification Number (TIN) (Social Security Number or Employer Identification Number) to a person or company paying you.

Main parts of the form

Part I: Personal information

  • Name
  • Business Name (if applicable, e.g., sole proprietor).
  • Address (mailing address).
  • Taxpayer Identification Number (TIN): SSN or EIN.

Part II: Certification

  • Sign and date the form to confirm the information is accurate.
  • Indicate if you are subject to backup withholding (and explain if applicable).
